Exxat at YSU: the system that holds your clinical record

Youngstown State University's 2026-27 graduate handbook names Exxat as the Centofanti School of Nursing's official system for clinical compliance, clinical placement and documentation, and requires every graduate nursing student to keep an active account for the whole program. Your compliance documents are due there at least 30 days before your first practicum. Older YSU pages still mention Medatrax and CastleBranch; read those as earlier systems for the same jobs.

What the handbook says Exxat is for

The handbook, revised in August 2026, gives Exxat three roles at once: clinical compliance, clinical placement and documentation management. The rule covers all graduate nursing students, and the account has to stay active from enrollment to graduation, not only during practicum terms. YSU's current RN to BSN and entry-level BSN handbooks refer to Exxat as well, so the school runs one system across its programs.

Clinical faculty assign your final clinical grade using preceptor evaluations, direct observation, clinical documentation and practicum logs, and much of that documentation lives in Exxat. Treat what you enter there as part of your academic record: the handbook lists falsified clinical documentation or misrepresented hours among the forms of unsafe practice.

What you do in Exxat

The handbook describes Exxat as your working file for everything clinical, from clearance to hours. These are the tasks it assigns to the system.

  • Upload the required clinical compliance documents and keep each one current.
  • Track expiration dates, so a certification or annual item never lapses during a practicum term.
  • Record clinical hours, patient encounters and other required practicum activities, which your preceptor verifies.
  • Find the immunization requirements, which the handbook places under School Resources on the Health Requirements tab.
  • Keep placement records in order: YSU requires an approved preceptor agreement on file before any clinical day.

The handbook also allows "other approved clinical documentation systems" for practicum records, so a course may name a different log. When the syllabus does, follow the syllabus.

The 30-day deadline and staying current

Unless YSU specifies otherwise, every required document has to be in Exxat at least 30 days before your first clinical practicum and then maintained for the rest of the program. A lapse is more than a paperwork problem: the handbook lists failure to maintain required compliance documents or certifications among the grounds for administrative withdrawal. Our suggested rhythm:

  1. Right after admissionSet up your Exxat account, read the requirement list under School Resources and start on anything slow, such as collecting old immunization records.
  2. Well before the cutoffOrder the drug screen and background check and book BLS, plus ACLS on the AGACNP track, so results arrive with time to spare.
  3. 30 days before the first practicumEverything uploaded and accepted. This is YSU's deadline, so treat it as the latest possible date rather than a target.
  4. Each year afterRenew the annual drug screen and background check, and keep BLS and ACLS current, between practicum terms rather than during one.

Medatrax and CastleBranch: older names on YSU pages

YSU's online FAQ, undated and still carrying COVID-era wording, describes an earlier setup. It routes clinical site and preceptor requests and hour tracking through Medatrax, says the "clinically ready" notice arrives in the Medatrax Mail Center, and sends clearance items to CastleBranch. The archived 2022-23 graduate handbook also used CastleBranch, including for fingerprinting. The 2026-27 handbook names Exxat for all of these jobs, so treat Exxat as current.

The FAQ's account of the paperwork still helps: a three-page packet holding the Student Preceptor Agreement and the Clinical Site Request, which you download, have signed by the site's signature authority and your preceptor, scan and upload. What to confirm with your clinical faculty

YSU's public documents leave several practical points open. Ask these early, ideally during orientation, so nothing surprises you the month before a practicum.

  • Where the Student Preceptor Agreement and Clinical Site Request are filed now, and who signs each part.
  • Whether a clinically ready notice still exists, and how it reaches you.
  • Which hour log your course uses, and whether your preceptor verifies hours inside Exxat or on a separate form.
  • Whether background checks and fingerprinting still run through a separate vendor, and which state's check you need if you live outside Ohio.
  • What the compliance vendors cost, since YSU publishes no figures.
  • How long YSU needs to review an upload, so you know how early to submit.

How the placement desk keeps Exxat moving

The slow part of Exxat is gathering what goes into it. For each practicum we assemble the preceptor's license, certification and experience details, the site's name and signature authority contact, and the dates, laid out the way the Student Preceptor Agreement and Clinical Site Request need them, ready for you to upload.

Questions YSU students ask

Is Exxat the same thing as Medatrax at YSU?

They are different products that YSU has named for overlapping jobs at different times. The undated online FAQ sends preceptor requests and hour tracking to Medatrax, while the 2026-27 graduate handbook, revised in August 2026, calls Exxat the official system for compliance, placement and documentation. Go with Exxat, and ask clinical faculty if an instruction still says Medatrax.

Do I have to pay for Exxat?

YSU's public documents give no price for Exxat and do not say how it is billed; the same goes for background checks and drug screens. Ask at orientation so you can budget. The one platform the handbook clearly makes a student purchase is Sentinel U, required for NP students, which the online tools guide covers.

Do YSU RN to BSN students use Exxat too?

Yes. The 2026-27 RN to BSN handbook says that when a practicum site requires a drug screen or background check, it has to be documented in Exxat, or a signed statement that you meet compliance through your employment must be on file. The RN to BSN guide covers that program's 60 practicum hours.

What happens if an Exxat item expires during a practicum?

The handbook requires compliance to be kept current throughout the program and lists failure to maintain required documents or certifications among the grounds for administrative withdrawal. Because clinicals can only happen once every requirement is met, a lapse can also pause your hours until it is fixed. Renew early and compare expiration dates with each term's end.

When should I set up my Exxat account?

As soon as YSU gives you access after admission. The FAQ asks students to begin site and preceptor requests at admission because approval can take many months, and the handbook requires an active Exxat account throughout enrollment. Setting up early also shows you the current requirement list, which YSU no longer posts publicly.
