Northwest Ohio practicums for YSU nurse practitioner students
Northwest Ohio spreads 1.33 million people over 18 counties, 12 of them nonmetro, from Toledo's hospitals to county hospitals in towns like Paulding and Coldwater. ProMedica places graduate students only under a contract with their school, Mercy Health's current NP process is unclear, and one health center network runs 50 sites from Toledo to Lima, so a YSU search here leans on community practices too.

Eighteen counties from Toledo to Lima and Sandusky
ODH's hospital preparedness Region 1 takes in Lucas, Wood, Fulton, Williams, Defiance, Henry, Paulding, Putnam, Van Wert, Allen, Mercer, Auglaize, Hancock, Seneca, Sandusky, Ottawa, Erie and Huron counties. The Census Bureau's July 2025 estimates total 1,332,700 residents, 423,347 of them in Lucas County.
USDA's 2023 codes class only six counties as metro: Lucas, Wood and Fulton around Toledo, Allen around Lima, and Ottawa and Erie near Sandusky. Hancock (Findlay), Seneca (Tiffin), Sandusky County (Fremont) and Huron (Norwalk) are nonmetro counties with towns over 20,000, and Paulding's code of 8 marks a town population under 5,000. Sandusky sits roughly 135 miles from Youngstown, Lima about 215.
ProMedica starts with a school contract
ProMedica's hospitals here include Toledo Hospital with the Russell J. Ebeid Children's Hospital, Flower Hospital in Sylvania, Bay Park in Oregon, Defiance Regional and Fostoria Community Hospital. According to its guidelines for nursing schools, read from a search index copy, every undergraduate and graduate student placement needs a valid contract between ProMedica and the student's nursing program. A program seeking one writes to ProMedica with its program type, accreditations and reason for the request, and placements are then settled with the liaison at each hospital.
The first move is YSU's, not yours: ask your clinical faculty whether YSU holds a ProMedica contract before you approach a ProMedica clinician. YSU cautions that a new agreement may still be in negotiation when the practicum is due to start, so keep a second site in view (affiliation agreements at YSU).
Mercy Health, Nationwide Children's Toledo and UTMC
Mercy Health, part of Bon Secours Mercy Health, runs St. Vincent Medical Center and St. Anne in Toledo, St. Charles in Oregon, hospitals in Tiffin, Willard and Defiance, and St. Rita's Medical Center in Lima. This site's research found no NP student page from Mercy Health, and how it takes NP students today is unclear, so treat these hospitals as landmarks and ask your clinical faculty about Mercy Health before counting on one.
The children's hospital at St. Vincent changed hands on March 1, 2022 and is now Nationwide Children's Hospital Toledo. Pediatric requests there follow the Columbus system's graduate rules: a clinical contract with your school, one assigned preceptor per term, an Ohio RN license and very few family NP spots (the Central Ohio guide has its request periods). The University of Toledo Medical Center has no public route for NP students from other schools.
Findlay, Lima, Sandusky and the county hospitals
Blanchard Valley Health System, with Blanchard Valley Hospital in Findlay and Bluffton Hospital, posts one general application on its student page for shadowing, precepting and clinical rotations, with no NP-specific rules. Lima Memorial's student page covers job shadowing and Firelands Regional Medical Center's covers its diploma RN school, so neither describes an NP route.
The rest of the region runs on community hospitals: Fisher-Titus in Norwalk, Wood County in Bowling Green, Memorial in Fremont, Bellevue, Magruder in Port Clinton, Fulton County Health Center in Wauseon, Community Hospitals and Wellness Centers in Bryan and Montpelier, and the Henry County, Paulding County, Van Wert County, Mercer County Joint Township and Grand Lake hospitals. None posts an NP student page, so ask each directly how it handles students.
Health centers and shortage designations
HRSA lists 111 active health center sites run by 10 organizations. Health Partners of Western Ohio alone has 50, spread over Toledo, Lima, Tiffin, Bryan, Defiance, Holland and Harrod. Family Health Services of Erie County has 18 around Sandusky and Norwalk, Neighborhood Health Association of Toledo 17, and the County of Erie 12, reaching Port Clinton and Kelleys Island. Community and Rural Health Services covers Fostoria, Fremont, Napoleon and Willard.
Paulding, Putnam, Van Wert, Mercer, Auglaize, Ottawa and Huron are whole-county primary care shortage areas. The low-income populations of Williams, Allen and Sandusky counties are designated too, and Lucas, Wood, Defiance, Hancock, Seneca and Erie have designations in part of the county. Lucas scores 22, the region's highest; Fulton and Henry have none. Mental health designations cover every county but Hancock, with Van Wert and Allen scoring 22.
Matching YSU practicums to the northwest
Family NP students fill NURS 7061 to NURS 7065 in primary care, aiming for YSU's suggested floor of 200 adult hours, 100 in pediatrics and 75 in women's health; acute care students fill NURS 7071 to NURS 7075 in acute settings (AGACNP practicums).
- Toledo: the region's densest cluster of hospitals for AGACNP hospital medicine, ICU and emergency hours, and Neighborhood Health Association and Health Partners sites for adult primary care.
- Lima and Findlay: St. Rita's, Lima Memorial and Blanchard Valley as acute care landmarks, with Health Partners' Lima sites for primary care.
- The rural west: in whole-county shortage areas such as Paulding, Van Wert and Mercer, one family practice may supply adult and pediatric hours together.
- Pediatric hours: with FNP spots scarce at Nationwide Children's Toledo, look at family practices and the region's 28 school-based health center sites.

Questions YSU students ask
Can I do YSU practicum hours at a ProMedica hospital?
ProMedica's guidelines require a contract between ProMedica and the nursing program for every graduate placement, and the program makes that request, not the student. Ask your clinical faculty whether YSU holds one. If a new contract would be needed, YSU notes that negotiations may not finish before your start date, so keep another site ready.
Where can an FNP student find primary care preceptors near Lima or Findlay?
Health Partners of Western Ohio has sites in Lima and Tiffin, and Community and Rural Health Services covers Fostoria and Fremont. Blanchard Valley Health System in Findlay takes precepting requests through its general student application. Independent family and internal medicine practices in Allen and Hancock counties round out the list.
Does Mercy Health take NP students in Toledo or Lima?
Its current process for NP rotations is unclear: this site's research found no NP student page from Mercy Health. Ask your clinical faculty how Mercy Health handles NP students before counting on St. Vincent, St. Anne, St. Charles or St. Rita's. Mercy Health's partnership with YSU is the nurse anesthesia program, not NP practicums.
I live in Michigan near Toledo. Can I train in northwest Ohio?
YSU's catalog and handbook accept Michigan residents, though its online MSN pages name Indiana instead, so confirm your status first. YSU requires an RN license valid where you complete practicum hours, and Michigan is not a Nurse Licensure Compact state, so Ohio hours need an Ohio license. See clinicals outside Ohio.
